Medical Social Worker - Home Health - (SW - HH)

This Medical Social Worker role is based in Riverside, California, a vibrant and growing city in the Inland Empire that blends historic character with modern amenities. The position is at a modern healthcare facility serving a broad and diverse patient population from Riverside and surrounding communities. As a Medical Social Worker, you will be an integral part of the interdisciplinary care team, working closely with physicians, nurses, case managers, and therapists to support patients and families through the medical care continuum. Daily responsibilities typically include conducting comprehensive psychosocial assessments, developing patient-centered care plans, and facilitating safe, timely discharges. You will provide counseling, crisis intervention, and emotional support for patients and families coping with acute and chronic illness, new diagnoses, complex social needs, or end-of-life issues. Additional duties may include coordinating resources and services such as home health, rehabilitation, hospice, community-based programs, and financial or housing assistance, as well as assisting with advance directives and advance care planning. The caseload will involve adult and possibly geriatric patients in medical-surgical units, specialty clinics, or other departments, depending on assignment. You will participate in interdisciplinary rounds, communicate regularly with the care team, and serve as a key advocate for patients navigating the healthcare system. The role requires strong organizational skills, clear documentation within the electronic medical record, and the ability to prioritize needs in a dynamic, fast-paced environment. Shifts may include daytime hours with potential for some evening, weekend, or holiday coverage based on department operations.
