License: Must have LCSW or LMSW license in hand. Certifications: BLS required; LCSW, LCSW-C, LMSW certifications required. Must-Have: - Acute hospital experience required. - Completion of at least one travel assignment required. - Flu vaccine required; no exemptions permitted. Description: The role requires a Social Worker or RN Case Manager with prior experience in acute rehabilitation and hospital settings. Candidates should have a strong background in discharge planning and interdisciplinary collaboration. The position involves care coordination for adult and geriatric populations. You will be responsible for conducting initial evaluations and developing plans of care to meet patient and caregiver needs. You will coordinate services to home care and equipment following discharge. Documentation of care delivery, goals, and patient response is essential. The role also involves facilitating discharge and contingency planning, ensuring quality care in a cost-effective manner. Experience with charting systems is required. Candidates should demonstrate the ability to collect and analyze patient information to determine medical necessity. Knowledge of cultural issues and patient vulnerabilities is crucial for effective intervention. The daily patient load includes an average of 18 patients. Collaboration with multidisciplinary teams is a key component of the position. Candidates should be prepared to attend daily interdisciplinary rounds with team members.
