Key Responsibilities of the Medical Social Worker- Perform psychosocial assessments
- Provide supportive counseling and care coordination
- Collaborate with RN case management and clinical team
- Coordinate discharge planning and service coordination for diverse populations
- Work with internal and external agencies for holistic care
Requirements of the Medical Social Worker- Master's degree in Social Work (MSW)
- Current Massachusetts Social Work licensure (LCSW/LICSW)
- Minimum of one year related experience in a hospital environment
- Ability to triage crises and work in high-urgency settings
- Flu vaccine required during onboarding
