Shift: 5x7-Hour Days (9:00 AM – 5:00 PM)
Hours: 35 hours/week
Guaranteed Hours: Dependent on facility requirements

Requirements:

  • Minimum 1 year of LMSW experience
  • Master's Degree in Social Work
  • Active Licensed Master Social Worker (LMSW) license issued by the New York State Education Department (NYSED) (Primary Source Verification required)
  • Experience in inpatient women's health is preferred

Job Responsibilities:

  • Provide psychosocial assessments and counseling to patients and their families.
  • Develop and implement discharge plans in collaboration with the healthcare team.
  • Coordinate referrals to community resources and support services.
  • Maintain accurate and timely documentation in the electronic medical record.
  • Advocate for patients and assist with crisis intervention and care coordination.
