State or federal-issued photo ID must be presented at the facility and worn as a photo badge at all times.

Agency staff must have completed the original or updated COVID-19 vaccine, or present a documented religious/medical exemption from their employer.

Overtime is not guaranteed. Every other weekend availability required. Reliable personal transportation is required - Uber is not a reliable option in this area.

Responsibilities

Assist residents with activities of daily living including bathing, dressing, grooming, and safe mobility.
Monitor and record vital signs, intake and output, and promptly report changes in resident status to the supervising nurse.
Deliver compassionate, person-centred care in a long-term or post-acute setting.
Maintain infection control and resident safety standards in all care activities.
Support nursing team and interdisciplinary care planning.

Requirements

Current, unencumbered LNA licence in Vermont - required.
Minimum 6 months of SNF/LTC experience preferred.
Current BLS/CPR certification.
Reliable personal transportation required.
