Job Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality patient care in a skilled nursing and rehabilitation setting. The LPN will support residents and patients with clinical needs while ensuring safety, dignity, and continuity of care.
This role requires strong clinical judgment, attention to detail, and the ability to work effectively within an interdisciplinary healthcare team.
Key Responsibilities
- Provide direct nursing care according to care plans and physician orders
- Administer medications and treatments safely and accurately
- Monitor, document, and report changes in patient condition
- Assist with admissions, discharges, and care plan updates
- Collaborate with RNs, physicians, and interdisciplinary care teams
- Maintain accurate and timely clinical documentation
- Follow infection control and safety protocols
- Respond to patient needs and clinical changes promptly
- Support patient and family education
Requirements
- Active Licensed Practical Nurse (LPN) license in the state of practice
- Completion of an accredited Practical Nursing program
- Current CPR/BLS certification preferred or required per facility
- Strong communication and clinical skills
- Ability to work long day shifts (06:00 AM 06:30 PM)
- Prior skilled nursing or rehab experience preferred
