REQUIREMENTS
Active Licensed Practical Nurse (LPN/LVN) License in good standing
Current BLS certification preferred
Previous long-term care, skilled nursing, rehabilitation, or related healthcare experience preferred
Strong clinical assessment and documentation skills
Excellent communication and interpersonal abilities
Ability to work effectively within an interdisciplinary healthcare team
KEY RESPONSIBILITIES
Provide direct nursing care and treatments to residents
Administer medications and monitor resident responses
Assess resident conditions and report significant changes to the healthcare team
Maintain accurate and timely clinical documentation
Assist in the development and implementation of individualized care plans
Collaborate with physicians, nurses, therapists, and support staff
Educate residents and families regarding care plans and treatment goals
Promote resident safety, dignity, and quality of life
Ensure compliance with facility policies, procedures, and regulatory standards
