Job Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality nursing care in a long-term care setting. The LPN will support residents with daily clinical needs while ensuring safety, dignity, and continuity of care.
This role requires strong clinical skills, attention to detail, and the ability to work effectively within an interdisciplinary healthcare team.
Key Responsibilities
- Provide direct nursing care to residents according to care plans and physician orders
- Administer medications and treatments as prescribed
- Monitor, document, and report changes in patient condition
- Assist with admissions, discharges, and care plan updates
- Collaborate with RNs, physicians, and care team members
- Maintain accurate and timely clinical documentation
- Follow infection control and facility safety protocols
- Respond to resident needs and emergencies appropriately
- Support resident education and family communication
Requirements
- Active Licensed Practical Nurse (LPN) license in the state of practice
- Completion of an accredited Practical Nursing program
- Current CPR/BLS certification preferred or required per facility
- Strong communication and patient care skills
- Ability to work evening shifts (03:00 PM 11:30 PM)
- Prior long-term care or skilled nursing experience preferred
