REQUIREMENTS
Active Licensed Practical Nurse (LPN/LVN) License in good standing
Current BLS certification preferred
Previous long-term care, rehabilitation, skilled nursing, or related healthcare experience preferred
Strong clinical assessment and medication administration skills
Excellent communication and documentation abilities
Ability to work effectively within an interdisciplinary healthcare team
Commitment to providing compassionate, resident-centered care
KEY RESPONSIBILITIES
Provide direct nursing care in accordance with physician orders and individualized care plans
Administer medications, treatments, and nursing interventions safely and accurately
Monitor resident conditions and promptly communicate changes to the healthcare team
Perform routine nursing assessments and maintain accurate patient records
Assist with the development and implementation of resident care plans
Collaborate with physicians, registered nurses, therapists, and support staff to coordinate care
Ensure compliance with infection-control procedures, safety standards, and regulatory requirements
Educate residents and families regarding care plans, treatments, and health needs
Promote resident comfort, dignity, safety, and quality of life
