Responsibilities

Provide direct patient care and carry out physician orders in compliance with NH nursing practice standards.
Administer medications and treatments accurately and in accordance with facility policies.
Assess, monitor, and document resident status; promptly report changes in condition to the RN or supervisor.
Collaborate with the interdisciplinary care team to develop and update individualised care plans.
Maintain professional, compassionate communication with residents, families, and colleagues.

Requirements

Current, unencumbered LPN licence in New Hampshire - required.
Minimum 1 year of experience in a long-term care or skilled nursing environment.
Familiarity with MAR and TAR documentation systems preferred.
Current BLS/CPR certification.
COVID-19 vaccination or documented employer-issued exemption required.
