Responsibilities
- Provide direct patient care under supervision in a long-term care setting
- Administer prescribed medications and treatments per orders and facility policy
- Monitor residents’ vital signs, assess changes in condition, and report to supervising staff
- Assist with activities of daily living and basic nursing procedures
- Document care and observations accurately in PCC; maintain patient privacy and charting integrity
- Collaborate with the health care team to implement and update individualized care plans
Requirements
- Current LPN license to practice in New Jersey
- Proficiency with PCC (PointClickCare) electronic health record
- Ability to work in a skilled nursing facility setting
- Availability for a schedule that includes 12-hour night shifts (7:00 pm–7:00 am) and 8-hour shifts (3:00 pm–11:00 pm)
- Strong communication and teamwork skills; ability to pass required background checks and screenings
