Responsibilities
- Provide comprehensive nursing care to patients in their homes, documenting interventions and outcomes.
- Perform assessments, implement care plans, administer medications as prescribed, and coordinate infusion therapy when needed.
- Collaborate with physicians, home health team members, therapists, and family/ccaregivers to coordinate effective home-based care.
- Monitor patient status, evaluate responses to interventions, and adjust plans of care accordingly.
- Maintain accurate, confidential clinical records and communicate data clearly in written and verbal form.
- Demonstrate independent judgment within scope of practice, manage time effectively, and adapt to variable schedules.
- Travel to patient homes using own vehicle; be prepared to work in varying weather and on-call as required.
Requirements
- Massachusetts LPN license (active) and Massachusetts Temporary Nursing Service Agency License (Nursing Pool) must be uploaded to profile prior to submission.
- Minimum one year of acute nursing care experience; hospital-based experience preferred.
- Experience in home health is required; certified home health with proficiency in OASIS is strongly preferred.
- BLS (American Heart Association) or Red Cross CPR certification required.
- Current driver’s license and auto insurance; traveler must have a vehicle and be able to drive in inclement weather.
- Knowledge of home care reimbursement and regulatory guidelines; ability to work independently with minimal supervision.
- Demonstrated interpersonal skills and ability to maintain confidentiality and professional boundaries.
- RTO/time-off requests must be documented in the profile at submission; ongoing scheduling will follow assignment policies.
