OverviewLicensed Practical Nurse (LPN) – Home Health Travel AssignmentLocation: Lynchburg, Virginia, United StatesStart date: 09/14/2026Duration: 8 weeksShift: 8 hoursGuaranteed hours: 40 per weekWeekly pay: $1,429–$1,460 per weekSetting: Home healthcareKey Highlights- Weekly pay: $1,429–$1,460 (per week)- Guaranteed hours: 40 hours per week- Contract duration: 8 weeks- Shift: 8-hour days- Start date: 09/14/2026- Work setting: Home healthcare travel assignment- Possible on-call dutiesResponsibilities- Administer skilled nursing care to patients in their place of residence under the supervision of an RN and according to the plan of care.- Coordinate care with the interdisciplinary team and communicate with patient/family; document outcomes and update clinical records accordingly.- Assist in planning, coordinating, and implementing patient care to ensure quality, cost-effective services.- Provide patient and family education on self-care techniques; assess responses to care and adjust interventions as needed.- Maintain infection control practices and safety standards in all patient encounters.- Assist the RN/physician with specialized duties, including end-of-life care when applicable, and support admissions, discharges, and transfers.- Participate in on-call duties if required and ensure timely communication of patient status to the care team.- Utilize electronic medical charting and time-keeping software (Homecare Homebase) for accurate documentation.Requirements- Active Licensed Practical Nurse (LPN) license in the practicing state (Virginia).- Current CPR certification.- Two years of LPN/LVN clinical experience, preferably in home health or hospice settings.- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.- Valid driver’s license with reliable transportation; current auto liability insurance.- Ability to read, write, and speak fluent English.- Proficiency with Homecare Homebase or willingness to learn electronic medical charting and time-keeping software.- Proof of hepatitis consent/declination and TB vaccination record.- Ability to work in a home healthcare setting and adhere to professional boundaries and safety standards.Benefits and Support- Guaranteed hours and weekly pay for the duration of the assignment.- 24/7 recruiter and travel support throughout the assignment.- Dedicated support to help with onboarding and ongoing needs during the travel assignment.Find more opportunitiesIf you’re interested in other travel nursing openings or additional locations, apply through Healthforce Healthcare LLC at https://healthforce.applytojob.com/apply to explore more opportunities.PAY DISCLAIMER RULE:Please note: Pay rates and guaranteed hours are estimates and may vary.
