This role involves working closely with nursing teams and healthcare providers to ensure safe, effective, and patient-centered care delivery.
Key Responsibilities
Provide direct nursing care to residents in a long-term care environment
Administer medications and treatments as prescribed
Monitor patient conditions and report changes appropriately
Document patient care accurately in medical records
Support implementation of individualized care plans
Assist with resident comfort, safety, and daily care needs
Follow infection control and facility safety procedures
Collaborate with interdisciplinary healthcare teams

Requirements
Active Licensed Practical Nurse (LPN) license required
Vermont license required or compact eligibility preferred
BLS certification preferred or required by facility
Long-term care or skilled nursing experience preferred
Ability to work weekend overnight schedule required
