Job Overview
We are seeking a Licensed Practical Nurse (LPN) to provide quality patient care within a long-term care and skilled nursing environment. The LPN will assist with patient treatment plans, administer medications, and support daily nursing operations during overnight shifts.
This role involves collaborating closely with nursing staff and interdisciplinary healthcare teams to ensure safe, compassionate, and efficient patient care delivery.
Shift Details
Shift Time: 18:00 06:30
Number of Shifts: 4 shifts per week
Start Requirement: ASAP starts preferred (within 2 weeks or less)
Key Responsibilities
Provide direct nursing care to residents and patients
Administer medications and treatments as prescribed
Monitor and document patient conditions and progress
Assist with patient assessments and care coordination
Support infection prevention and patient safety practices
Collaborate with RNs, physicians, and healthcare teams
Maintain accurate clinical documentation and reports
Respond promptly to patient needs and clinical concerns
Requirements
Active Licensed Practical Nurse (LPN) license
Experience in long-term care or skilled nursing preferred
Strong patient care and clinical support skills
