Job Overview
We are seeking a Travel Licensed Practical Nurse (T-LPN) for a 13-week contract assignment in St. Johnsbury, Vermont. The LPN will provide direct patient care under the supervision of registered nursing staff while supporting residents in a skilled nursing environment.
This role requires strong clinical competency, adaptability, and the ability to work across multiple units while delivering high-quality patient care during overnight shifts.
Key Responsibilities
Provide direct patient care under RN supervision
Administer medications and treatments as prescribed
Monitor patient conditions and report changes promptly
Assist with activities of daily living (ADLs)
Maintain accurate and timely patient documentation
Support wound care and basic nursing procedures
Ensure compliance with infection control and safety standards
Collaborate with interdisciplinary healthcare teams
Provide patient and family education within scope of practice
Work flexibly across assigned units as needed
Requirements
Active Licensed Practical Nurse (LPN) license in the state of Vermont or compact/eligible license
Graduate of an accredited Practical Nursing program
Current CPR/BLS certification (if required by facility standards)
Minimum 1 year of clinical experience preferred in skilled nursing or long-term care
Experience using PointClickCare (PCC) preferred
Ability to work overnight 12-hour shifts
Flexibility to work across multiple units within the facility
Strong clinical, communication, and teamwork skills
