SHIFT DETAILS

• Shift Time: 6:45 AM 3:15 PM
• Meal Break: 30 Minutes
• Day Shift

GUARANTEED HOURS & PAY

• Guaranteed Hours: 40 Hours Per Week
• Pay Rate: $36.33 - $42.39 per hour

REQUIREMENTS

• Active Licensed Practical Nurse (LPN/LVN) License in good standing
• Current BLS certification preferred
• Previous long-term care, rehabilitation, or skilled nursing experience preferred
• Strong clinical assessment and patient care skills
• Excellent communication and documentation abilities
• Ability to work effectively within an interdisciplinary healthcare team
• Commitment to delivering high-quality resident-centered care

KEY RESPONSIBILITIES

• Provide direct nursing care to residents in accordance with physician orders and care plans
• Administer medications, treatments, and treatments documentation accurately
• Monitor resident conditions and communicate changes to supervising nurses and physicians
• Assist with assessments and implementation of individualized care plans
• Maintain accurate and timely patient documentation
• Collaborate with RNs, physicians, therapists, and support staff to coordinate care
• Ensure compliance with infection-control policies and safety regulations
• Support resident comfort, dignity, and quality of life
• Promote a safe and therapeutic environment for residents and staff

• Guaranteed 40 hours per week
• Competitive pay rate of $36.33 - $42.39 per hour
