Harris Hill Center in Concord, NH is seeking an experienced Travel Licensed Practical Nurse (LPN) for a 15-week contract assignment.
The LPN will provide quality nursing care to residents, administer medications and treatments, monitor patient conditions, maintain accurate documentation, and collaborate with the healthcare team to ensure safe and compassionate care.
Responsibilities
Provide direct nursing care according to resident care plans.
Administer medications and treatments as ordered.
Monitor residents and report changes in condition.
Maintain accurate nursing documentation and medical records.
Collaborate with RNs, physicians, CNAs, and other healthcare professionals.
Support resident safety, comfort, and quality of life.
Follow facility policies and applicable healthcare regulations.
Assist with care coordination and other nursing duties as assigned.
Qualifications
Current and active LPN license.
Previous LTC, SNF, rehabilitation, or post-acute care experience preferred.
Current BLS certification preferred.
Strong clinical, communication, and organizational skills.
