• Provide direct nursing care to residents in a skilled nursing/long-term care setting.
• Administer medications and treatments as prescribed.
• Monitor resident conditions and report changes to the healthcare team.
• Maintain accurate patient documentation and records.
• Collaborate with RNs, physicians, and interdisciplinary staff to ensure quality patient care.
• Follow facility policies and infection control standards.

Requirements

• Active LPN License (VT or Compact License if applicable)
• Minimum 1 year of Long-Term Care (LTC) or Skilled Nursing Facility (SNF) experience preferred
• Current BLS Certification
• Strong clinical and communication skills
• Ability to work evening shifts and complete a 13-week assignment
