Position Overview
We are seeking an experienced and adaptable Travel LPN (Long-Term Care) for an evening shift position in Goffstown, NH. In this role, you will provide ongoing chronic disease management, daily nursing care, and routine assessments for residents in a long-term care setting. We are looking for a nurse who emphasizes person-centered care, patient safety, and seamless coordination with a multidisciplinary team to maintain our residents' quality of life.
Key Responsibilities
Monitor vital signs, administer medications, and document patient responses accurately.
Perform wound care, dressing changes, and routine clinical procedures per individual care plans.
Assess changes in resident conditions and promptly communicate updates to RNs, physicians, and family members.
Assist with activities of daily living (ADLs) and rehabilitation activities as directed.
Maintain accurate medical records in the Electronic Health Record (EHR) system while strictly adhering to safety protocols.
Qualifications & Requirements
Education: Graduate of an approved practical nursing program.
Licensure: Current LPN license in the state of New Hampshire (or an active Compact/NLC license).
Certifications: Current CPR / BLS certification.
Experience: 1+ years of experience in long-term care or skilled nursing facilities.
Skills: Strong communication skills with families and team members, adaptability, and proficiency in EHR documentation and care planning.
Preferred Qualifications
Hands-on experience with wound care management or palliative care principles.
