Job Title: Licensed Vocational Nurse (LVN)

Location: State Supported Living Center

Assignment Duration: 6 months

Schedule: Full-time, 40 hours/week

Shift Hours: 2:00 PM – 10:00 PM

Tentative Start Date: 08/01/2026

Position Overview:

We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to provide high-quality, patient-centered care at a Long-Term Care Facility within a State Supported Living Center. Under the supervision of Registered Nurses (RNs) and physicians, you will be a vital part of a team committed to ensuring the safety, comfort, and well-being of our residents.

Key Responsibilities:

  • Deliver direct nursing care, including monitoring vital signs, administering medications, and assisting with daily living activities.
  • Observe and promptly report changes in patient conditions to the supervising RN or physician.
  • Assist with patient admissions, discharges, and transfers to ensure seamless transitions.
  • Perform basic wound care, dressing changes, and specimen collection.
  • Maintain accurate, timely documentation of patient care activities and interventions.
  • Educate patients and their families on care plans, medication regimens, and post-discharge instructions to promote understanding and compliance.
  • Collaborate effectively with interdisciplinary teams to support positive patient outcomes.
  • Uphold cleanliness, safety, and organization within the care environment to meet infection control standards.

Qualifications:

  • Valid, unencumbered state-issued LVN license.
  • Completion of an accredited practical nursing program.
  • Minimum of 2 years of clinical experience in a healthcare setting.
  • Current Basic Life Support (BLS) certification.
  • Strong clinical skills with a focus on patient safety and care.
  • Excellent communication and interpersonal abilities to foster a supportive patient environment.
