Weekend: Every Other Weekend

Job Summary

We are seeking an experienced LPN/LVN to join the Ready for Discharge Unit (RDU). The LPN will provide direct patient care for medically stable patients awaiting discharge, collaborate with the multidisciplinary team, support discharge planning, and ensure safe, high-quality patient care.

Requirements

  • Active Oregon LPN License
  • Minimum 2 years of acute care experience
  • BLS Certification
  • Experience with Epic charting preferred

  • Acute Care / Medical-Surgical nursing
  • Patient assessment and direct patient care
  • Discharge planning and patient education
  • Medication administration
  • Documentation and EMR (Epic)
  • Care coordination with multidisciplinary teams

Note: Acute care experience and an active Oregon LPN license are required.
