Licensed Practical Nurse (LPN) – Night Shift

Connected Health is hiring a compassionate and dependable Licensed Practical Nurse (LPN) for one of our trusted partner facilities in Wakefield, Nebraska. This opportunity is based in a skilled nursing and assisted living setting where patient-centered care and teamwork are top priorities.

Schedule:

• Night Shift
• 3x12 schedule
• Rotating weekends

Responsibilities:

• Provide direct nursing care to residents in skilled nursing and assisted living
• Administer medications and treatments as ordered
• Monitor residents’ conditions and report changes appropriately
• Collaborate with nurses, providers, and care staff to ensure quality care
• Maintain accurate documentation and support resident safety and comfort

Qualifications:

• Active Nebraska LPN license required
• Minimum 1 year of skilled nursing experience required
• Strong clinical and communication skills
• Compassionate, reliable, and team-oriented mindset
