Key Responsibilities

• Perform nursing assessments on all patients within 8 hours of admission and develop individualized plans of care in collaboration with the patient, family, and interdisciplinary team
• Provide accurate, timely documentation of all assessments, interventions, and patient responses in the medical record in accordance with policy
• Participate in the patient rehabilitation program, utilizing the nursing process to assess, plan, implement, and evaluate patient care with an emphasis on patient and family education
• Initiate discharge planning from the day of admission and provide patient teaching in anticipation of discharge
• Communicate pertinent changes in patient status to attending physicians, charge nurses, and care team members to support continuity of care and patient safety
• Provide direction, training, and supervision of Licensed Practical Nurses, Certified Nursing Assistants, and orientees, delegating appropriately and utilizing effective problem-solving techniques

Required Qualifications

• Active Registered Nurse (RN) license in the state of Michigan
• Demonstrated competency in nursing assessment, care planning, and clinical documentation
• Ability to supervise and delegate to ancillary nursing staff effectively
• Strong commitment to patient safety, including safe patient handling, infection control, and fall prevention practices
• Adherence to standards of personal appearance, attendance, and punctuality per facility guidelines

Preferred Experience / Skills

• Prior experience in an acute rehabilitation or inpatient hospital setting
• Experience participating in interdisciplinary team conferences and rehabilitation care programs
• Demonstrated commitment to ongoing professional development and continuing education
