Job Title: Case Management Social Worker Profession: Social Services Specialty: Licensed Masters of Social Work (LMSW) Duration: 13 Weeks Shift: Day Hours per Shift: 8 Experience: Minimum of 1 year in case management with acute care experience, discharge planning, and care coordination. License: Washington State Licensed Masters of Social Work (LMSW) Certifications: BLS Must-Have: Strong preference for diverse case management experience including Med Surg. Must be able to work in all areas of the facility. Description: The role encompasses providing comprehensive support to patients through effective discharge planning and care coordination. Candidates will conduct biopsychosocial assessments and coordinate services to ensure patient needs are met upon discharge. You will be responsible for documenting care delivery, plans, and goals. The position requires collaboration with various internal and external team members. Experience in adult protective services, advance directives, and guardianship is essential. Knowledge of Medicaid and Medicare regulations will be beneficial. Candidates must exhibit strong communication skills and the ability to perform high-risk interventions. The clinician will work with diverse patient demographics, including adults and geriatrics. Floating between units may be required based on patient care needs. The expected average caseload varies; flexibility to handle between 18-30 patients is necessary. Candidates must be prepared to work weekends and commit to holiday shifts. Previous charge experience in a medical or social work setting is preferred. Familiarity with EPIC charting systems is required.
