Job Title: Licensed Masters of Social Worker Profession: Social Services Specialty: Case Management Duration: 8 Weeks Shift: Day 4x10-Hour Hours per Shift: 10 Experience: 1 year License: Washington License (LMSW or LCSW) required Certifications: BLS required Must-Have: - Acute Care Experience - Discharge Planning/Care Coordination Experience - Diverse Case Management Experience in Med Surg, LLOS, ED Description: This position requires a Licensed Masters of Social Worker. Strong preference for candidates with experience in acute care and care coordination. Candidates should have a minimum of one-year experience in case management. Acceptance of first-time travelers is available. Weekends are required during rotation, specifically every other weekend. The role may involve floating to various units within the facility, including areas with high patient volume. Candidates must possess experience with Epic charting systems. Responsibilities include conducting initial evaluations and coordinating services post-discharge. Collaboration with interdisciplinary teams is an essential part of the role. Candidates must demonstrate knowledge of cultural issues and patient vulnerabilities. The average caseload is 18-24 patients, potentially rising to 30 on weekends. Documentation of care delivery and participation in quality care coordination is expected. Emergency Department experience is a plus. Applicants should have a thorough understanding of legal aspects, such as Power of Attorney and Advance Directives.
