You will be expected to manage a caseload of up to 25 patients. Patient ratios during the day are approximately 1:35. The role includes focusing on discharge planning and care coordination. You will participate in the development of treatment plans. You must work weekends every other week and be available for holiday coverage. This role requires effective documentation according to the plan of care. You will educate patients and caregivers to facilitate discharge. Interdisciplinary support includes various healthcare teams and services. The ideal candidate should have strong communication and collaboration skills.
