Travel Medical Social Worker - $2,376 per week in Orange, CA

Job Title: Medical Social Worker Profession: Social Services Specialty: MSW (Masters of Social Work) Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 16:30 Experience: Minimum 2 years of experience required License: CA state license required Certifications: MSW required Must-Have: - Previous charge experience - Experience with case management and discharge planning - Proficiency in documentation and charting systems Description: Start date: ASAP. First time traveler is acceptable with manager review. Weekend requirements to be discussed during interview. Travel experience is required. Pending licenses are not accepted. No more than 7 days of requested time off. The role involves working in acute care case management and discharge planning. The candidate will be responsible for case load management of 20-25 patients. Responsibilities include initial assessments within 24 hours and coordination of discharge plans. The candidate must work closely with a multidisciplinary team to ensure continuity of care. Experience with various patient diagnoses is essential. Knowledge of HCBS, Medicare, and Medicaid is preferred. The role requires facilitation of patient and caregiver education. Candidates will coordinate post-acute care needs and arrange transfers to physician appointments. Crisis intervention and patient advocacy skills are necessary. Candidates must demonstrate an understanding of the NASW Code of Ethics and standards of home health care. Willingness to float between facilities is a requirement. Commitment to working three of the five holidays is necessary. Candidates will need to provide references covering relevant experience. All communication devices required for the role will be supplied by the client. Modules are non-billable and must be completed pre-start and annually. Completion of modules will be self-reported and recorded.
