Job Title: Social Worker Profession: Social Services Specialty: MSW (Masters of Social Work) Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 16:30 Experience: Minimum 2 years of acute medical social work experience, performing both social work and discharge planning License: California state license required; pending license accepted but must be licensed upon start Certifications: MSW, LCSW either/or MSW Must-Have: - Knowledge of drug and alcohol abuse treatment options - Experience with patients experiencing homelessness and those with mental health histories - Ability to organize and participate in family meetings - Experience in the development of treatment plans Description: Start date is as soon as possible. Floating is not expected. First-time travelers must be screened. The patient ratio during days is 1:25. Typically, one full weekend shift every four weeks is required, subject to department needs. The daily case load will be up to 25 patients. The social worker will be responsible for care coordination, collaborative care, and developing plans to meet patient and caregiver needs. The role includes discharge planning and participating in care conference activities. Additional skills required include needs assessment, ordering durable medical equipment, and short-term acute care inpatient medical social work. Knowledge of advance directives, POLST, protective services processes, and related areas is essential. Travelers are expected to float between all facilities within the state. Refusal to float may result in assignment termination and/or forfeiture of guaranteed hours. References are required, covering a minimum of one year within the last three years. All travelers are expected to work three of the five holidays. Providers will be provided communication devices necessary for job performance at no cost. Modules are non-billable, and time spent on modules must be self-reported. Completion of modules is recorded in transcripts.
