This role involves conducting biopsychosocial assessments and developing plans of care to meet patient and caregiver needs. The Social Worker will assist patients with navigating various processes and provide education to facilitate discharge. Participation in care conference activities is essential. Knowledge of cultural issues and experience with high-risk intervention patient vulnerability is required. The Social Worker must work collaboratively with a multi-disciplinary team to ensure continuity of care. Experience with drug and alcohol abuse treatment options is important. The Social Worker should be capable of organizing and participating in family meetings. Responsibilities include documentation of care delivery, goals, and patient responses. The Social Worker will also manage cases involving child protective services and adult protective services. All interactions should be conducted in accordance with professional ethics and standards. Weekend coverage may be required as part of this role.
