Key Responsibilities

• Perform nursing assessments on all patients within 8 hours of admission and develop individualized plans of care in collaboration with the patient, family, and interdisciplinary team.
• Provide accurate, succinct, and timely documentation of all assessments, interventions, and patient responses in the medical record in accordance with policy.
• Participate in patient rehabilitation programs, utilizing the nursing process of assessing, planning, implementing, and evaluating care with a focus on patient and family education.
• Initiate discharge planning from the day of admission and provide patient teaching in anticipation of the discharge date.
• Communicate pertinent information regarding changes in patient status to attending physicians, charge nurses, and other care providers to ensure continuity of care and patient safety.
• Provide direction, training, and supervision of Licensed Practical Nurses, Certified Nursing Assistants, and orientees, delegating appropriately and utilizing effective problem-solving techniques.

Required Qualifications

• Current, active Registered Nurse (RN) licensure in the state of Alabama or compact licensure eligibility.
• Demonstrated ability to perform comprehensive nursing assessments and develop individualized care plans.
• Competency in safe patient handling, infection control, and adherence to hospital safety policies and procedures.
• Ability to supervise and delegate to ancillary nursing staff effectively.
• Strong documentation skills with the ability to maintain accurate and timely medical records.

Preferred Experience / Skills

• Experience in a rehabilitation or long-term acute care setting.
• Prior charge nurse experience or demonstrated leadership in a clinical environment.
• Commitment to ongoing professional development through continuing education and self-study.
