About the Role
We’re seeking an experienced Medical Social Worker (MSW/LCSW) to join a fast-paced acute care inpatient setting. This role focuses heavily on case management and discharge planning, working closely with interdisciplinary teams to ensure safe, timely, and effective patient transitions of care.
⚠️ Important: This is NOT a behavioral health or ED-focused role — candidates must have recent acute care hospital experience.
Key Responsibilities
- Manage a caseload of 20–23 patients in an acute care setting
- Complete initial biopsychosocial assessments within 24 hours
- Coordinate discharge planning and post-acute care services
- Collaborate with physicians, nurses, and insurance case managers
- Arrange DME, home health, rehab, and long-term care placements
- Provide crisis intervention and patient/family counseling
- Assist with appeals, denials, and utilization review
- Educate patients and caregivers for safe discharge
- Document all care using EPIC EMR (Required)
✅ Requirements
- Minimum 2 years of experience (recent acute care within last 3 months REQUIRED)
- At least 1 full year of inpatient hospital experience (MANDATORY)
- MSW Required | LCSW Preferred
- BLS Certification (Required)
- EPIC experience – HARDSTOP (Required)
- Travel experience required (No first-time travelers)

- ❌No first-time travelers accepted
- ❌Behavioral health or ED-only experience will NOT be considered
- Schedule may vary; facility operates 7 days/week
- May require floating to nearby Laguna facility
- Max 7 days RTO allowed
