3B Healthcare Inc. is seeking a travel nurse RN Care Manager Director for a travel nursing job in Greenbrae, California.

Job Description & Requirements

Role-: Director of Care Coordination 

 

Job Duties-: 

  • Demonstrated experience leading interdisciplinary clinical/operational programs, staff, performance improvement, and regulatory or accreditation readiness.
  • Advanced knowledge of case management, care coordination, utilization management, discharge planning, patient progression, and transitions of care in an acute-care environment. 

Job Details-: 

  • 13 weeks contract
  • Day shifts
  • Shift schedule- Monday to Friday
  • EPIC charting system
  • Guaranteed hours- 40 hours per week | 8 hours per day
  • Possible on call- TBD 

Required-: 

  • BSN is a must.
  • Master's degree in nursing or health administration or business administration
  • Certified Case Manager (CCM or ACM)
  • Must have 5 years of leadership/administrative experience in Case Management, Care Coordination, Utilization Management in an acute healthcare setting is mandatory.
  • Active California RN License
  • BLS
