Job Description – Nurse Practitioner (NP) | PACE Program
Weekly Pay: $4946

Location: Grand Junction, CO
Contract: 13 Weeks
Schedule: 4 x 10-hour Day Shifts (On-site)

Responsibilities

Provide primary care services for elderly patients enrolled in the PACE program.
Diagnose and treat acute and chronic medical conditions.
Perform preventive care, health assessments, and urgent care visits.
Evaluate patients in the clinic, PACE Day Center, and in their homes as needed.
Collaborate with the Medical Director and interdisciplinary care team.
Coordinate referrals to specialists and community providers.

Requirements

Minimum 2 years of Nurse Practitioner experience.
Experience in Primary Care, Geriatrics, Family Practice, Internal Medicine, or Hospice/Palliative Care preferred.
Active NP license and prescriptive authority (per state requirements).
Strong clinical assessment and patient management skills.
