Sawgrass Healthcare (A Datson360 Company) is seeking a Nurse Practitioner Primary Care for a travel job in Grand Junction, Colorado.

Nurse Practitioner (PACE) – Grand Junction, CO

Contract: 13 Weeks
Schedule: 4x10 Hour Day Shifts (On-Site)
Setting: Primary Care Clinic with In-Home Visits as Needed

Requirements:

Current, unrestricted Colorado APN & RXN license
National certification as FNP, AGPCNP, or related NP specialty preferred
DEA registration (or eligibility to obtain) and Controlled Substance License
Current BLS/CPR certification
Valid driver's license, acceptable driving record, and proof of automobile insurance
Experience in primary care, geriatrics, or chronic disease management preferred

Job Highlights:

Provide comprehensive primary care for frail seniors enrolled in the PACE program
Diagnose and treat acute and chronic conditions
Perform health assessments in the clinic, PACE Day Center, and participants' homes
Collaborate with an interdisciplinary care team to develop individualized care plans
Coordinate referrals and ongoing care with community providers
Focus on helping seniors remain safely in their homes and communities
