Registered Nurse (RN) Team Lead - Skilled Nursing Facility
Job Summary
Join a dedicated healthcare team at a 120-bed Skilled Nursing Facility in Montour Falls, New York. This facility operates three distinct units, each accommodating up to 40 residents and managed by an RN Manager. As an RN Team Lead, you will play a pivotal role in delivering high-quality patient care and providing leadership on the evening shift.
This position involves direct patient care, including comprehensive assessment, monitoring, and documentation of resident conditions. You will be responsible for coordinating care with an interdisciplinary team, advocating for patients, and ensuring adherence to evidence-based practices. The role requires strong critical thinking, delegation, and prioritization skills to manage a unit effectively, maintaining a staffing ratio of 1 RN, 2 LPNs, and 3-4 CNAs for 40 residents.
Key Responsibilities
Provide direct patient care, including comprehensive assessment, monitoring, recording, and reporting of changes in patient condition.
Demonstrate strong time management, critical thinking, delegation, and prioritization skills in leading the nursing team.
Collaborate effectively with the interdisciplinary care team to coordinate patient care plans and promote positive outcomes.
Advocate for patients' needs and ensure their well-being and safety.
Practice ethically and professionally within the scope of nursing practice, promoting evidence-based practices (EBP) and best practices.
Maintain clinical competence specific to the care area and complete all organization-required education within stated timeframes.
Document patient care accurately and thoroughly in the Electronic Medical Record (EMR).
Participate in unit councils and staff meetings to contribute to continuous quality improvement.
Ability to assume LPN Med Cart responsibilities as needed and staffing permits.
Required Qualifications
Active Registered Nurse (RN) license in the State of New York.
Ability to lift up to 50 lbs. and perform physical tasks associated with patient care.
Ability to sit or stand for long periods and physically operate equipment required for the job.
