Registered Nurse (RN) Team Lead - Skilled Nursing Facility
Job Summary

This contract role is situated within a Skilled Nursing Facility (SNF) and focuses on providing comprehensive nursing care while demonstrating leadership in daily operations.
The Registered Nurse (RN) Team Lead is responsible for direct patient care, including assessment, monitoring, and reporting patient conditions, alongside critical thinking, delegation, and prioritization. This role requires strong collaboration with the interdisciplinary team and patient advocacy.
Candidates must demonstrate flexibility to support patient care needs across various roles, including the ability to perform medication administration duties at an LPN scope of practice when staffing requires.
Key Responsibilities
- Provide direct patient care, including comprehensive assessment, monitoring, recording, and reporting changes in patient conditions.
- Lead and coordinate patient care activities, demonstrating strong time management, critical thinking, delegation, and prioritization skills.
- Collaborate effectively with the interdisciplinary care team, participating in care planning and promoting a cohesive care environment.
- Act as a patient advocate, ensuring ethical and professional practice within the scope of nursing.
- Promote Evidence-Based Practice (EBP) and best practices related to patient care.
- Maintain clinical competence specific to the care area, including completion of all organization-required education within stated timeframes.
- Coordinate care with the interdisciplinary team to ensure continuity and optimal patient outcomes.
- Communicate effectively with patients, families, and colleagues, and document appropriately and thoroughly in the Electronic Medical Record (EMR).
- Participate actively in unit councils and staff meetings to contribute to unit improvement and team development.
- Demonstrate flexibility by floating to other units as staffing permits.
- Perform physical tasks including lifting up to 50 lbs., and standing or sitting for extended periods.
- Operate all required medical equipment safely and proficiently.
Required Qualifications
- Current and unrestricted Registered Nurse (RN) license.
- Ability to function in an LPN capacity for medication administration when staffing requires.
- Ability to perform the physical requirements of the role, including lifting up to 50 lbs.
- Commitment to working every other weekend and every other holiday.
- Ability to float to other units as needed.
