Position: RN – Hospital Discharge Planner / Home Care Coordinator
Facility: Johns Hopkins Bayview Medical Center
Schedule:Monday–Friday, 8:30 AM–5:00 PM
Weekend: Weekend coverage may be required; Saturday/Sunday, 8:30 AM–4:00 PM
Contract:13 Weeks
Experience:2+ years of current Home Care, Discharge Planning, or Case Management experience
License:Maryland RN license required
Certifications:BLS
EMR:Epic
Work Setting:On-site at Johns Hopkins Bayview Medical Center
Role responsibilities:
Coordinate patient discharge and home-care services
Arrange necessary medical equipment such as walkers and commodes
Connect patients with appropriate home-care services and community resources
Educate patients and families about their care needs
Facilitate continuity of care when patients transition from the hospital to home
