REGISTERED NURSE – AMBULATORY CARE

Crownpoint Service Unit, Navajo Area Indian Health Service

 

Crownpoint, New Mexico (Crownpoint Healthcare Facility, Thoreau Health Center, Pueblo Pintado Health Center)  |  Full Time  |  Approx. 2,600 hours/year (Monday–Friday day shift; tour of duty 0730–1600 or 0800–1630 as assigned)

 

Position Overview

This position provides Ambulatory Care Branch (outpatient) registered nursing services for the Crownpoint Service Unit in support of the Navajo Area Indian Health Service. The role covers full-scope adult and pediatric case management, patient care coordination, and clinical nursing support across Crownpoint Healthcare Facility and its two satellite clinics, Pueblo Pintado Health Center and Thoreau Health Center. The contract consists of a one-year base period with one one-year option period.

 

Key Responsibilities

  • Provide full-range adult and pediatric case management services in Ambulatory Care and/or Women and Children's Services.
  • Apply the nursing process to deliver individualized, goal-oriented care and develop, implement, and evaluate patient care coordination plans.
  • Collaborate with primary care providers, clinic nurses, public health nursing, case management, and social services to ensure timely, cost-effective care.
  • Manage clinic visits and follow-up care through coordination with multi-disciplinary teams, including chart reviews.
  • Arrange and facilitate patient-care conferences with providers, patients, and families; interpret as needed.
  • Coordinate with Purchased & Referred Care (PRC) staff on outside testing/results follow-up; participate in care team planning rounds.
  • Document all care rendered in the Electronic Health Record (EHR)/RPMS system in accordance with facility standards.
  • Follow facility policies and procedures and the standards of The Joint Commission (TJC), OSHA, and IHS.
  • Maintain compliance with HIPAA, the Privacy Act, and NAIHS information security requirements.

Qualifications & Experience

  • Minimum 36 months of nursing experience, including at least 2 years in ambulatory care/case management or an equivalent specialty.
  • Current, valid, unrestricted RN license in a U.S. state, D.C., Puerto Rico, or a U.S. territory.
  • Current Basic Life Support (BLS) certification required; PALS and ACLS recommended.
  • Ability to pass required background investigation (Level I/II), fingerprinting, and IHS onboarding/credentialing requirements.
  • Current immunization documentation (MMR immunity, Hepatitis B series or declination, varicella immunity, Tdap, annual TB screening, seasonal influenza vaccination).
  • Fitness-for-duty certification from a licensed physician.
  • Strong English communication skills (read, write, speak) and cultural awareness/respect for Navajo and American Indian/Alaska Native patients and community.
  • Basic computer proficiency, including use of Electronic Health Record/RPMS systems.

Compensation

This is a remote/rural assignment located on the Navajo Nation. Pay is structured as follows:

  • Taxable Hourly Pay Rate: $41–$47/hour, depending on experience and certifications.
  • Per Diem (non-taxable, where applicable): up to $178/day for lodging and meals (based on the GSA per diem rate for McKinley County, NM), plus a one-time $500 travel allowance paid at assignment start.
  • Blended Pay Rate (Taxable + Per Diem combined): $65–$73/hour, depending on eligibility, experience, and certifications.

Government housing is available only if a vacancy exists; otherwise housing/travel arrangements are the responsibility of the employee.
