Facility: Jackson Purchase Medical Center Location: Mayfield, KY Department: Case Management Shift: 08:00–17:00 Schedule: Days Guaranteed Hours: 40/week Contract: 91 weeks Openings: 1 Orientation: Weekly on Mondays
Required Qualifications
RN with ADN
Minimum 5 years of experience
Case Management experience
ACM certification
BLS
Discharge Planning experience
Medical Necessity experience
Experience with difficult placement patients
Must be 60 miles or greater from the facility to qualify as a traveler
Current weekend coverage also includes 3–4 hours in-house on weekends/holidays
Dress Code
Scrubs – any color
No hoodies
No open-hole Crocs
