Job Summary

We are seeking a seasoned Travel RN Case Manager with strong acute care experience in Case Management and Discharge Planning. The RN Case Manager will coordinate patient care, facilitate safe and timely discharges, assess patient needs, and collaborate with physicians and multidisciplinary teams to ensure appropriate utilization of healthcare resources.

Responsibilities

• Provide comprehensive case management services in an acute hospital setting.
• Perform patient assessments and develop individualized plans of care.
• Coordinate patient care, admissions, transfers, and discharge planning.
• Collaborate with physicians, nurses, social workers, and other healthcare professionals.
• Evaluate patient needs and coordinate post-acute services, including DME and community resources.
• Manage prior authorizations and benefits eligibility as applicable.
• Apply admission criteria and determine appropriate levels of care.
• Support disease management and continuity of care.
• Monitor patient progress and identify barriers to discharge.
• Ensure compliance with CMS, Department of Health, DRG, Joint Commission, Core Measures, and National Patient Safety Goals.
• Maintain accurate documentation in Epic EMR.
• Work with patients across Emergency Department, ICU, Med-Surg, and Surgical Services.

Required Qualifications

• Minimum 2 years of Acute Care Case Management/Discharge Planning experience.
• Hospital experience required.
• Behavioral Health experience does NOT qualify toward the required acute care experience.
• Seasoned travel experience required – First-time travelers are NOT accepted.
• Active California RN license required.
• Pending CA license may be considered but must be active by start date.
• Epic experience required.
• Strong knowledge of care coordination, discharge planning, utilization review, and acute care workflows.
• Ability to work independently and effectively within a multidisciplinary team.
