Job Summary

We are seeking an experienced RN Case Manager / Care Manager to provide comprehensive care coordination, utilization management, and discharge planning services in an acute care hospital setting. The Care Manager will collaborate with patients, families, physicians, nursing staff, and multidisciplinary teams to ensure appropriate, cost-effective care and successful transitions across the continuum.

Responsibilities

• Perform comprehensive patient assessments and identify appropriate care coordination needs.
• Develop and implement individualized plans of care based on patient needs and clinical status.
• Coordinate admissions, continued stays, transfers, and discharge planning.
• Determine medical necessity using evidence-based guidelines and established criteria.
• Collaborate with physicians and multidisciplinary teams to facilitate appropriate patient care.
• Coordinate post-acute services, including DME, home health, rehabilitation, and other community resources.
• Support disease management and identify barriers to safe and timely discharge.
• Monitor patient progress and modify care plans as needed.
• Maintain accurate documentation in Epic and comply with hospital, regulatory, and HIPAA requirements.
• Apply knowledge of CMS, DRGs, CPT coding/billing, and Department of Health regulations.
• Provide care management support across acute care units, including ICU, Med-Surg, SDU/PCU/IMC, and Observation.

Required Qualifications

• Minimum 2 years of acute medical Care Manager experience.
• Strong experience in Care Coordination and Discharge Planning.
• Travel experience required; first-time travelers may be considered after screening.
• California RN license required; pending license may be accepted but must be active by start date.
• Previous experience with Epic EMR required.
• Strong knowledge of acute hospital case management/utilization review practices.
• Experience with medical necessity determination and evidence-based guidelines.
• Strong communication, organization, critical thinking, and care coordination skills.
