Position Summary:We are actively seeking an experienced Travel Registered Nurse (RN) – Case Manager to join a highly respected acute care hospital in Santa Monica, CA. This 13-week travel assignment is ideal for an RN with recent acute care case management experience who excels in care coordination, discharge planning, and multidisciplinary collaboration. If you're passionate about improving patient outcomes while working in a fast-paced hospital environment, this is an excellent opportunity to advance your travel nursing career with a leading healthcare organization.
 

Key Responsibilities:

  • Coordinate patient care throughout hospitalization.
  • Perform comprehensive patient assessments.
  • Develop and implement individualized discharge plans.
  • Collaborate with physicians, nurses, social workers, and interdisciplinary teams.
  • Arrange DME, SNF placement, home health services, and transportation.
  • Participate in family meetings and treatment planning.
  • Ensure compliance with CMS, HIPAA, Joint Commission, and hospital regulations.

Requirements:

  • Active California RN license
  • BLS by AHA
  • COVID card (Medical/Religious exemptions are accepted)
  • Minimum 2 years of recent Acute Care RN Case Management experience
  • Recent Care Coordination experience within the last 3 months
  • Previous Travel Nursing experience required
  • Epic EMR experience required
  • Experience with discharge planning and care coordination in an acute hospital setting
