RN Case Manager - Hospice
Coordinate patient care from pre-admission through discharge and post-discharge, supporting continuity of care and efficient progression through hospitalization.
Key Responsibilities
Collaborate on the development, implementation, evaluation and revision of case management plans.
Serve as a liaison among patients, families, nurses, physicians, the multidisciplinary team and healthcare payers.
Consult with the healthcare team on individual patient needs.
Coordinate care transitions to optimize patient outcomes.
