Job Summary:
We are seeking an experienced RN Case Manager to provide inpatient case management services across the continuum of care. The RN Case Manager will assess patient needs, develop individualized care plans, coordinate services with multidisciplinary teams, monitor outcomes, and support appropriate discharge planning and resource utilization.

Responsibilities:

• Assess and reassess patients to identify clinical, psychosocial, financial, and discharge needs.
• Develop, implement, monitor, and evaluate individualized plans of care.
• Coordinate patient care with physicians, nursing staff, Social Services, and other healthcare professionals.
• Support appropriate transitions and discharge planning across acute care, home health, rehabilitation, hospice, and other community settings.
• Apply critical thinking and clinical judgment to determine appropriate interventions and resources.
• Monitor medical necessity, quality outcomes, and appropriate utilization of healthcare resources.
• Communicate effectively with patients, families, physicians, and multidisciplinary teams.
• Assist with problem-solving, conflict resolution, and coordination of complex patient needs.
• Maintain accurate documentation and comply with regulatory, organizational, and patient safety standards.

Requirements:

• BSN required.
• Graduate of an accredited nursing program.
• Current California RN License required.
• Minimum 2 years of acute clinical nursing experience required.
• Minimum 2 years of Case Management experience required.
• CCM or ACM certification preferred.
• Travel nursing experience and California healthcare experience strongly preferred.
• Cerner and Epic experience preferred.
• Knowledge of MCG, MIDAS+, Medicare regulations, Conditions of Participation, and Joint Commission standards preferred.
• Strong assessment, organization, time management, communication, critical-thinking, and problem-solving skills.
• Ability to work independently and effectively in a multidisciplinary team.

Patient Population/Units:
May be assigned across acute care units, including Med-Surg, Med-Tele, Surgical, ICU, and potentially ED.

Preferred Candidate Profile:
The manager is seeking someone who can hit the ground running. Candidates with travel experience, California experience, or ideally both are strongly preferred. ED experience would be a major asset.

Important: Candidates must have 2+ years of acute clinical nursing experience and 2+ years of Case Management experience, along with a BSN and active California RN license.
