Job Details:
Job Title: TRAVEL- RN Case Management (924) / MC Fort Worth / 8A-4:30P
Location: Fort Worth, TX
Setting: Hospital
Pay Range: 1553.19 - 1630.85
Schedule: 08:00 - 16:30
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: most recruited for: (Travel RNs, local CNAs and sitters); Must have: candidates without these skills will not be considered for the role. • Acute Care Case Management Experience (at least 1 year)
Responsibilities:

  • Provide case management services to patients in the neuro step-down unit.
  • Collaborate with a multidisciplinary team to develop and implement patient care plans.
  • Utilize Meditech and Careport EMR systems to document patient interactions and treatment plans.
  • Manage patient flow and ensure compliance with unit policies.
  • Support floating assignments across varied case management roles as needed.
