Acute Rehabilitation Case Manager – Simplified Job Details
Facility: Virginia Mason Franciscan Health Rehabilitation Hospital
Location: Washington
Position: Acute Rehabilitation Case Manager
Start: ASAP
Shift: 8:00 AM – 4:30 PM
Schedule: Monday–Friday
Guaranteed Hours: 40/week
Weekend: None
Call: None
Openings: 1
Reason: Contractor replacement
Requirements
- Case Management experience — required
- 2–3+ years preferred, but flexible based on experience
- Must have a current professional healthcare license as one of the following:
- RN
- Social Worker
- Respiratory Therapist
- Physical Therapist
- Occupational Therapist
- Speech-Language Pathologist
- Must have appropriate Case Management experience
- Acute hospital experience with adult patients is preferred
- Strong verbal and written communication
- Basic Excel, Word, and Outlook skills
- Good organization, time management, and analytical skills
- Ability to work independently and as part of a multidisciplinary team
- Must maintain patient confidentiality
Certifications
Case Management certification is preferred, not strictly required. Examples include:
- CCM – Commission for Case Manager Certification
- ARN – Rehabilitation Nursing certification
- ACM – Association of Rehabilitation Management
- ANCC RN-BC in Case Management
Facility & Unit
- 60-bed freestanding acute rehabilitation hospital
- Patient ratio: Approximately 1:15
- Patient population: Adults, including patients recovering from stroke, medical debilitation, and post-surgical conditions
- EHR: UDS PRO, MediTech, and Pinpoint
- Average rehabilitation length of stay: approximately 12 days
Key Responsibilities / Experience
The candidate should be comfortable with:
- Case management in an acute rehabilitation/IRF setting
- Utilization Review
- Supporting interdisciplinary team/family conferences
- Working with nursing, therapy, physicians, patients, and families
- Discharge/LOS planning and forward planning
- Balancing patient advocacy with appropriate use of healthcare resources
- Communicating professionally during difficult insurance or healthcare discussions
IRF team-conference experience is preferred, but the facility will provide assistance/training for this requirement.
