Position Summary
Seeking an experienced RN Case Manager to support patients throughout their acute care hospitalization and manage the discharge planning and transition process.
Responsibilities
- Coordinate patients throughout their acute care hospitalization
- Manage discharge planning and transitions of care
- Perform utilization management and quality screenings
- Provide inpatient case management services
- Collaborate with patients, families, and the healthcare team to support appropriate care transitions
Requirements
- 2 years of acute care experience strongly preferred
- Inpatient Case Management experience required
- Case Management and Managed Care experience required
- BSN required
- Active RN license required
- CCMC certification preferred
