Requirements
Degree in Nursing or related field
Active professional license in the state
Minimum of 2 years of case management experience
Experience with electronic medical records (EMR)
Responsibilities
Assist in the collaboration, development, implementation, revision, and reporting of the case management program.
Act as a liaison between patients, families, healthcare teams, and other stakeholders to optimize outcomes.
Coordinate a continuum of care from pre-admission through post-discharge.
Ensure efficient patient and family progression through hospitalization.
Consult with the healthcare team on specific patient items
