LanceSoft is seeking a travel nurse RN Case Management for a travel nursing job in Chesapeake, Virginia.

Job Requirements - 

Schedule: 5x8 Days 8am-430pm, weekdays, some weekends, holidays, no overtime without supervisor approval

Minimum Requirements 

Active Virginia or Compact RN license in hand at the time of submission; no exceptions
Must be proficient in Epic; no exceptions

Important Candidate Requirements

Candidates must meet the following qualifications to be considered:

•  Recent experience in an adult acute care hospital setting with discharge planning
•  Minimum 5 years of recent acute care Case Management experience (hospital setting)
•  Less than a 6-month gap since their last acute care Case Management role
•  Experience in a Level I, II, or III acute care hospital
•  Previous patient census of approximately 15–25 patients
•  Rated 4/5 or 5/5 across all Case Management functions, including knowledge of CMS regulations
•  Epic proficiency is required (no exceptions)
•   Previous travel experience strongly preferred. First-time travellers will only be considered if they have extensive acute care Case Management experience.
•  Acute care discharge planning
•  Utilization Review (UR) experience required
•  Ability to effectively manage a high-volume, fast-paced patient population

Required Certifications 

BLS through the American Heart Association

Preferred Certifications 

CCM (Certified Case Manager)
ACM (Accredited Case Manager)
